The Practice has reviewed its benzodiazepine prescribing and based on current national guidelines will no longer prescribe benzodiazepines or sedatives (such as diazepam) to patients for fear of flying. This will be effective as of 1st January 2025.
Please deliver your form to the reception desk. The GP will complete this form, but there will be a charge as this is Non-NHS Service. Please ask at the receptionist how much you should pay.
Your form should be ready for uplift with 5 to 10 working days (this maybe longer during periods of annual leave or illness).
